A round shapely buttock is a sign of youth and beauty. As we age, the buttocks get smaller, more flat, and begin to droop. A flat buttock even in a young patient, makes a person look older, and less sexually attractive. Conversely: a round full buttock even in a older patient, makes the patient look younger and more sexually attractive. How a buttock looks is highly dependent on the fat distribution and genetics of a patient. Unfortunately many people simply genetically do not place their fat in their buttocks and/or have smaller buttock muscle mass. While African-Americans tend to have larger buttocks, Asians and Phillipinos tend to have flat, smaller buttocks. Increasingly women and men are desiring larger buttocks and buttock enhancement. Fortunately there are several ways to enhance buttock appearance, and make them more round, and/or larger.
There are many benefits to buttock augmentation. Larger buttocks can help balance other areas of the figure, for example large breasts or wide shoulders or hips. Larger buttocks look sexier, and make a patients look younger. Buttock augmentation can help lift buttocks that are sagging. Most importantly patients look and feel more attractive. Often this leads to more confidence, and improved self-esteem. In general it leads to an improved quality of life. Patients often feel this is one of the best investments they have ever made.
Buttocks can be enlarged with a buttock implant or by fat transfer to the buttocks. Fat transfer to the buttocks is performed by taking fat from one area and moving it to the buttocks. People often call this a Brazilian Butt Lift. Silicone implants can also be used to enlarge the buttocks, in a fashion similar to breast augmentation. Buttocks can be enlarged by using either a buttock implant or by using a persons own fat. This technically called buttock fat transfer but most people call it a Brazilian Butt Lift. A Brazilian buttock lift involves taking fat from one area and moving it to another area. This fat is obtained by performing liposuction. Technically this is called “fat transfer to the buttocks,” and the buttocks are enlarged by the person’s own fat. When possible it is better to use the patient’s own fat to enlarge the buttocks than a buttock implant. Usually the fat is taken from the flanks (“love handles,”) and abdomen, and other places as needed. The main advantage is that the buttock is enlarged with a person’s own fat. This means that the patient will never need another surgery. Another very big advantage is that each area we liposuction to harvest the fat is improved. This means the patient’s entire figure is improved, not just the buttocks. This can result in a much sexier look, than just augmenting the buttocks alone. The major disadvantage is that the fat transfer is more variable than the buttock implants, meaning sometimes the buttock enlargement is not as large or round as was wanted. This can usually be corrected with additional liposuction and fat transfer. Another disadvantage is that in order for this to work, a patient must have enough fat to transfer. The operation will not work on very thin patients.
The simple answer to this is no. The number and distribution of fat cells is relatively constant throughout life. This means that you don’t gain any fat cells if you get fatter, simply the fat cells you have expand like balloons. Also if you lose weight, you do not lose any fat cells, they simply shrink. This means that if we move fat cells from one area, such as the waist, and place them in the buttocks, the buttocks will forever be enlarged and the waist will forever be smaller. These proportions will remain no matter what the patient’s weight is. This means that after a Brazilian Butt Lift, if a patient gains weight it will go straight to the buttocks, and if the patient looses weight, the last place it will go from is the buttocks.
This procedure is usually done under general anesthesia. Fat transfer to the buttocks is done by first performing liposuction to harvest the fat. (It is helpful to read the section on liposuction located in this website.) We suction fat from the pre-determined areas; usually the flanks (waist) and abdomen, and often other areas such as the upper back, bra-line area or arms and inner thighs. After the fat has been harvested, we then spin the fat down to get pure fat. The pure fat is then injected into the buttocks through 3 small poke holes. One inside the gluteal crease (“butt crack,”) and one small poke along the lower outer portion of each buttock. We always place more fat than is needed because only about half of the injected fat lives. However, we cannot place more fat than the skin will hold. What this means is that as we inject the fat, the skin will get more tight, until it becomes so tight that no more fat will fit. When this happens we have to stop injecting fat, even if the patient wants more fat or larger buttocks. After surgery the patient is placed in a special compression garment that helps hold the fat in place. Some of the injected fat will die. Most of the fat that is going to die does so in the first 2 to weeks. After about 4 to 6 weeks, whatever fat is left will be permanent. This procedure will lead to permanent enlargement of the buttocks.
Usually the pain is quite manageable with the prescribed pain medications after buttock enhancement surgery (Brazilian Butt Lift). The pain usually subsides quite quickly after the first few days. After Brazilian butt lift, most of the pain is from the liposuctioned areas, and not very much from the fat transfer to the buttocks. Dr. Smith prefers that patients not work for at least one week, but most patients may return to work after 7 to 10 days. Also it is important to sleep prone (on your belly,) for 3 to 4 weeks after surgery, and to avoid sitting on the buttocks as much as possible for the first 2 to 3 weeks after surgery. As with every surgery, we do not want the patient to do any heavy exercise for 6 to 8 weeks after surgery. Patients may return to light exercise 4 to 5 weeks after surgery.
